Please print, fill-out, and bring the Patient Information Form and Policies/Notices to your first appointment.  Also, please bring any of the three following questionnaires as they apply.

Patient Information Form- confidential patient contact and insurance
       information

    Policies and Notices
        -Financial Agreement- allows us to file your insurance and clarifies payment
          obligations
        -Consent to Treat- clarifies the scope of examination and treatment options
        -Privacy Notice- outlines privacy policy and disclosure of private health       
          information

    Headache Questionnaire- if headache is your primary concern

    Back Pain Questionnaire- if back or low back pain is your primary concern

    Neck Pain Questionnaire- if neck pain is your primary concern


Insurance Benefits

The Insurance and Billing Department of Cole Pain Therapy Group is committed to helping patients with their insurance needs.  Our friendly staff is happy to consult with you and investigate your chiropractic benefits.  The doctors of Cole Pain Therapy Group accept the  following insurance carriers:

In the event of an auto accident or work injury, Cole Pain Therapy Group will also file your med pay or file workman’s compensation.
